Milestones
This book uses page references rather than folio references. They
appear in the margins, keyed to specific spots in the text
by asterisks. Insert the <MILESTONE> tag in the text at
the appropriate place and omit the asterisk(s). They
look like this:

Notes
... appear in the margins, keyed to places in the text
with standard footnote-type reference numbers. Treat
